Identifying Drug-Drug Interactions in Diabetic Patients with Chronic Kidney Disease: Pharmacist’s Interventions

Drug-drug interactions (DDIs) can negatively affect patients ‘therapeutic outcomes. Therefore, our aim was to evaluate DDIs among diabetic with chronic kidney disease (CKD). To achieve this aim, objectives were identify in CKD admitted the Medicine Department of tertiary care hospital and pharmacist’s interventions managing these patients. A prospective observational study conducted over 6 months. The pharmacist performed a medication chart review, identified by using Lexicomp® drug interaction. informed prescribers regarding occurrence DDIs, all pharmacist`s classified according Pharmaceutical Care Network Europe. Overall 307 total 119 an average 2.6 per patient. most (205, 66.7%) belonged interaction risk-rating category C, which indicates that required close monitoring patients’ therapy avoid any potential adverse outcome. needed be managed considering modification (risk-rating D) avoiding combination X) accounted for 19.2% 14.0% detected interactions, respectively. Interactions between Furosemide–Insulin (43, 14.0%), Amlodipine–Calcium carbonate/vitamin D3 (35, 11.4%) found commonly DDIs. delivered different types prescribers, ranged from outcome stopping great proportion (87%) accepted prescribers.
